Observation

During the onsite complaint inspection it was observed that staff were not providing active supervision while on the playground. The intake report stated that children made the program staff aware of the injuries. After interviewing staff, it was noted that staff were not aware that the child was sitting on top of the monkey bars and did not ask her to get down before she jumped off.
